Personal History

As part of the hiring process, applicants will complete a Personal History Questionnaire (PHQ) — a detailed form that helps us get to know your background, experiences, and qualifications. This step ensures transparency and helps our hiring team make informed decisions as you progress through the process. The questionnaire covers topics such as employment history, education, driving record, and other important information relevant to serving as a police officer.
